TL:DR, interviewed for job at a bank HQ in NYC, got job, requested 2 hour lunch breaks on Thursdays for ongoing health-related treatment (job offered 1 hour lunches), and I agreed to makeup by taking 30 hour lunches rest of the week since I bring my own food, HR agreed upfront but behind closed doors freaks, rescinds offer, and the notations made are absolute BS. Health issue was also not a surprise, listed disability on job app, mentioned it during interviews, and at my first day provided documentation, Worked 2 weeks, past Friday I'm given given email to leave. Got accommodations in writing, but HR rep who approved it quit
Update 1: I'm glad that my friend from the company has been supportive, and our collective boss called me to say he was sorry and that he knows something isn't right. He said from how HR tells the narrative I sound like someone who should never have been hired in the first place. They really made me out to be a terrible applicant, more than a terrible employee and to him that means they dropped the ball and messed up. I told him it's fine, I've lost my interest in the position anyhow.

Company cold-called me. They wanted to interview me again and this time they would come to my job to interview me to be considerate. I told them I needed photos for security. Sure enough it's the same folks who gave me hell before.
I told security in my building that I was being harassed, gave them the photos and said that anyone matching these photos was to be escorted out of the building, and police were to be called if they caused any issue.
I'm currently awaiting background checks and the likes for a federal job but, government hiring takes time, they said it could be a year+. So as I wait I've been unsatisfied with my current company and am looking elsewhere. I sent my resume out, back in January and luckily got a few job interviews. One was for a Director of QA Automation here in the city at a bank's HQ.

Feb 27th email giving me details on the role from HR and asking for a good time to talk
March 19th HR Screening on the phone
March 20th HR got back, setup a follow-up with an engineering panel via bluejeans
March 25th 1st engineering interview, white-boarded the whole time
March 29th HR emails me saying good feedback
April 4th I have a 2nd engineering interview again bluejeans
May 3rd I'm invited for a on-site interview the next business day, forgot about them, and was turned off to be emailed near the end of the day.
May 6th on-site interview, mostly repeated questions and white-boarded the same questions from the first 2 video interviews
June 6th my second on-site interview, lot of management style questions+technical ones
June 13th is my third on-site interview, HR oriented questions+technical ones
July 1st is my final on-site interview, I swear it was just the first 3 interviews combined.
July 3rd HR makes an offer
July 5th I negotiated the salary+benefits, my only issue was that it was actually not what was originally told to me by HR on the screening, the original figure was given on the screening, as well as the first email that included details on the role, and also during my final on-site interview.
July 25th I accepted the offer and dropped my documents, including a formal request for my accommodation, needing a longer break on Thursdays, this accommodation would be for 6 months and my doctor is only 6 blocks away. HR gives me a signed document securing my accommodation.
August 5th I begin, the HR representative who signed my documents had quit, and HR calls me in, they asked my to repeat what I had told them before and just went over the documents, I checked all the boxes and they had everything they needed. I left feeling satisfied, nobody gave me any trouble
August 16th HR sends an email saying peace, and I emailed my friend/fellow VP of Engineering because the last time I was fired was like, when i worked at staples as a high schooler? It was a little surprising and hurtful. I'd done contract work for the company in the past, so it was a matter of reputation as well as future income. He saw what they had put for the termination
The document basically went into detail on how difficult of an employee I was
-They said that during the interview process I came across as rude and hostile on multiple occasions and that raised flags
-They noted that I had negotiated a salary higher than the range they gave me and that it wasn't cleared by management, I was lowballed when given an offer from them after my final interview and had them honor the range given in the interviews, and the email. They never mention that they had I guess given me an incorrect range?
-They noted the accommodations I needed weren't backed by official documents and that they asked me twice for copies and that I never gave, I gave once, then the person quit, so when called in we went over and I saw myself they have the documents
-They said I wanted 2 hour breaks for treatment every day, instead of Thursdays, despite me having a signed document stating otherwise
-They state the HR rep who approved my accommodation had already put in her notice and that since she was on her way out her judgement was compromised and she was more lenient with incoming employees
-They noted I had also falsified my references, that was false,
During the interview process one of my references passed away, so I replaced her on my resume and sent them an updated copy highlighting the change
Then Another references changed companies and with it their direct business number, I updated that after my final offer
they noted these down as "severe discrepancies"
-They noted that my accommodation would cause undue undue hardship and financial drain to do the company
-They said that on my last day I didn't even show up, when I absolutely did.
-They said that I made a mistake when referring to HP Quality Center, HP Loadrunner, HP Release Control apparently these go by different names because they're under different publishers? And that showed I had lied about the depth of my knowledge of SW Testing tools
-My paycheck reflects their BS fairytale, I'm paid for 6 hours a day instead of 8 and no doubt they'll say it's because I get 2 hour lunches a day.
I don't take this personally. My reputation with the company is probably shaky but I'm secure enough that I know I'll be fine, and contract work and my network remains intact as far as the rest of the city is concerned. If this was early on in my career I'd be worried.
The part that gets me. I did everything correctly. I'm trying to be objective and see it from their side, and the only thing I can think of that scared them, was having a disabled director. I can admit, I work for one of the top 4 banks now, disabled engineers do not get hired, minorities rarely get hired, my department I make it a responsibility to be inclusive and diverse, and I call out any discrimination, but never been on the receiving end. I have mental disabilities that are maintained and I don't disclose, but my current health issue is cardiac related and I didn't think that disclosing a physical disability was going to be a problem.
